GSK Vaccine Access Program

The GSK Vaccines Access Program provides GlaxoSmithKline vaccines to adult
applicants who meet eligibility requirements.
Currently Cervarix, Twinrix, Boostrix, EngerixB and Havrix are available through the program.
To enroll, you must meet these eligibility requirements:
- Be an adult 19 years or older or a female between 19 and 25 for Cervarix.
- Meet program income guidelines.
- Live in one of the 50 states or the District of Columbia.
- Have no third party coverage for vaccines.
With the help of your prescriber, there are three easy steps to apply for free vaccine covered through the program:
- Your prescriber must be registered in the program.
- Your prescriber then must fax your completed and signed application
along with your income documentation to 1–877–822–1555. Once approved, you will be eligible to receive vaccine for up to one year.
- Once enrolled, you can receive subsequent doses of vaccine by having your prescriber complete the
Dose Authorization Request Form.
